Privacy policy

How this website collects, uses and stores personal information. Last updated 30 July 2026.

Who this policy applies to

This policy covers Northland Knee & Hip (NZBN 9429053770165) and this website. It explains what happens to information you send us through the site. It sits alongside, and does not replace, the obligations that apply to your clinical records, which are governed by the Privacy Act 2020 and the Health Information Privacy Code 2020.

What we collect through this website

The website has no user accounts, no login and no e-commerce. The only information we collect is what you choose to type into an enquiry form:

  • Your name
  • Your email address and phone number
  • Who referred you, or your practice name if you are a referrer
  • The general category of your enquiry
  • Whatever you write in the message field

We ask you not to send detailed clinical information or urgent matters through the enquiry forms. If you do include health information in a message, it is treated as health information under the Health Information Privacy Code.

How form submissions are handled

Enquiry forms on this site are processed by Formspree, a third-party form service. When you submit a form, the content is transmitted to Formspree, stored on Formspree’s servers, and emailed to the practice. Formspree’s servers are located outside New Zealand.

Once an enquiry reaches the practice it is handled like any other enquiry to the rooms, and if it becomes part of your care it is recorded in your clinical record.

Cookies and analytics

This website does not set analytics or advertising cookies, and does not use Google Analytics, advertising pixels or third-party tracking scripts. Fonts are served from this website rather than from a third party.

If website analytics are added in future, this policy will be updated to say what is collected and how.

How we use your information

We use the information you send through this site to:

  • Respond to your enquiry
  • Arrange or manage an appointment
  • Communicate with your referrer where that is relevant to your care

We do not sell your information, and we do not use it for marketing without your consent.

Who we share it with

Information may be shared with the parties involved in your care and its funding: your GP or referrer, the hospital where any procedure is performed, other treating clinicians, ACC, and your health insurer where you have asked us to deal with them. Beyond that we disclose information only where the law requires it.

Security

This website is served over HTTPS. Reasonable technical and organisational measures are taken to protect information held by the practice. No transmission over the internet can be guaranteed to be completely secure, which is why the enquiry forms ask you not to send detailed clinical information.

Your rights

Under the Privacy Act 2020 you have the right to ask what personal information we hold about you, to ask for a copy of it, and to ask for it to be corrected if it is wrong. To make a request, contact the practice at admin@nkh.co.nz or phone 027 555 5210.

If you are not satisfied with how we have handled your information, you can complain to the Office of the Privacy Commissioner at privacy.org.nz, or, for health information, to the Health and Disability Commissioner at hdc.org.nz.
